Tummy Tuck (Abdominoplasty)

Abdominoplasty helps to flatten and shape the abdomen and waist by removing excess fat and skin and tightening the abdominal wall muscles. By tightening the underlying muscles, Dr. Ghavami creates an "internal corset" effect that produces a beautifully balanced waist line and abdominal silhouette. The best candidates have pockets of fat or loose skin that haven't responded well to diet and exercise. The procedure can also be useful for women with stretched skin and muscles from pregnancy. After the effects of pregnancy, a breast lift may also be needed and the combination is commonly known as a "mommy makeover."

Recovery time ranges from two weeks to a few months, depending on the extent of the tummy tuck and other factors. Most people typically go back to work in 2-3 weeks. With a balanced diet and regular exercise, results are long-lasting and extremely satisfying to patients.

Tummy Tuck - Continued

Most often drains are needed to help remove the excess post-surgical fluid so it does not collect under the newly positioned skin. Dr. Ghavami uses specialized techniques that help surgical drains to come out much sooner than normally expected (3-5 days on average versus the usual 1-2 weeks).

Abdominoplasty does leave a permanent scar stretching between the hips, although it is designed with care so that can be concealed in the undergarment or bikini line. Some patients qualify for a "mini-tummy" tuck combined with liposuction which reduces the length of the scar. Dr. Ghavami will determine if this is right for you.

We also encourage patients who intend to lose a lot of weight to wait before undergoing the procedure, since skin in the abdominal area typically loosens after substantial weight loss. Sometimes liposuction can be done first to help in this process as a first stage and the excess skin removal and muscle tightening can be removed at a second stage.

Liposuction (Liposculpture)

The most popular cosmetic surgery procedure in the U.S., liposuction sculpts the body by removing undesirable fat that has not responded to diet and exercise from areas such as the abdomen, waist, hips, buttocks, thighs, knees, chin, cheeks, neck and upper arms. Local anesthesia or general anesthesia may be used, and the procedure can last anywhere from one to five hours depending on the amount of fat being removed.

There are several different methods surgeons use when performing liposuction, including ultrasound-assisted lipectomy (UAL) and suction-assisted lipectomy (SAL). In both types, fluid is injected into the area to be treated. Then a tiny incision is made in as inconspicuous a place as possible, and a cannula (small tube) breaks up the fat layer and suctions it out. Liposuction is an effective way to contour the body and is often used in combination with procedures that remove excess skin in places like the abdomen, arms, and thighs. Liposuction can also help create desirable facial and neck contours.

Some refer to certain techniques as liposculpture, which is in essence the same as liposuction. The most important point, is that techniques (such as UAL vs. SAL or depth of suction) can be varied according to the location being suctioned and your body requirements.

Body Contouring after Massive Weight Loss-Lower Body Lift and Other Procedures

Candidates for body lifts typically have loose (inelastic) skin on many or most areas of the body as a result of significant weight loss due to diet, exercise or more commonly, bariatric surgery (lap band or gastric bypass). Because of this, the procedure is often combined with others such as breast lifts, arm lifts and liposuction to retain a proper body proportion. Many times, for the safety and comfort of the patients, the numerous procedures are performed in separate stages. Body lifts tighten and tone the abdomen, back, buttocks, hips and thighs. In addition, an "auto-augmentation" of the buttock fat can be used to add fullness to a buttock region that has become deflated over time.

Fat transfers tend not to work well in massive weight loss patients as there is not enough fat to transfer. Therefore, islands of tissue are created that can be rotated into areas that need fullness. These are just some of the recent advances in this highly specialized aspect of modern plastic surgery.

Thigh Lift

A thigh lift is performed to remove excess skin and fat from the upper legs. Candidates typically demonstrate loose, inelastic skin after extreme weight loss due to diet, exercise, or bariatric surgery such as lap band and gastric bypass procedures. Because of this, the procedure is often combined with liposuction to help in contouring the thigh.

Depending on the amount of skin laxity and shape of the thighs, Dr. Ghavami will tailor the location of the incision to what is most beneficial to your body type. As discussed earlier, massive weight loss patients often require other procedures to balance their face and body and complete the weight loss rehabilitation process. This includes procedures such as lower body lift, breastlift, facelift and/or armlift done in planned stages.

In a medial thigh lift (inner thigh lift), the surgical incision is made obliquely at the inner thigh along the line where the leg meets the groin or vertically along the inner aspect of the thigh. In a lateral thigh lift (outer thigh lift), the incision is made from the groin around the back of the body, so that the buttocks are lifted along with the thigh in combination with a lower body lift or extended tummy tuck.

Arm Lift/ Arm Reduction (Brachioplasty)

After substantial weight loss, weight gain or simply from the effects of aging, skin on the upper arms can become inelastic, loose and flabby. Arm lifts, also known as brachioplasty, remove that excess skin and fat. Patients who have significant amounts of fat may want to consider combining the procedure with liposuction. This procedure can also be combined with a breast lift (mastopexy) on the same day. The extra tissue under the armpits and arms can be used on occasion to augment breasts that have become deflated from significant weight loss. This is known as "auto-augmentation" which is a technique used to augment various parts of the face and body after the soft tissue deflation that occurs after massive weight loss.
