Breast Augmentation

Breast augmentation is one of the most frequently performed cosmetic surgery procedures in the U.S. It can give women with small or unevenly-sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. The procedure may be combined with others such as a breast lift when needed. Depending on the skin quality, amount of breast sagging, and other factors, this can be done either as a single procedure or as staged procedures to reduce the chances for re-operation.

Implants are silicone shells filled with either saline (sterile salt water) or silicone gel (recently re-approved by the FDA for use in the U.S.). They are placed behind each breast, underneath either breast tissue or the chest wall muscle. The procedure typically lasts less than one hour and is performed with general anesthesia. After surgery the patient's bustline may be increased by one or more cup sizes. Dr. Ghavami will take detailed measurements of your breast during your consultation to determine the implant size that would look best for your body. Dr. Ghavami uses special surgical techniques that limit bleeding, tissue injury, and therefore post-op pain. Most patients are able to return to their life in less than two days after the surgery. These are remarkable advances. Drains are not used. No excessive bandaging or wraps are needed, just a surgical bra.

Breast Lift (Mastopexy)

A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have sagged as a result of weight loss, pregnancy, loss of the skin's natural elasticity or simply the effects of aging and gravity. The procedure can also reduce areolar size (the dark skin surrounding the nipple), and it can be combined with breast augmentation for added firmness.

Breast lifting is performed under general anesthesia, and lasts from one and a half to four hours. Dr. Ghavami makes every attempt to reduce the locations and size of the incisions to limit unnecessary scar formation. Depending on specific breast characteristics, a patient may qualify for "short scar" techniques. This will be determined after measurements are taken during the consultation. Dr. Ghavami performs periareolar (donut shaped), vertical scar (lollipop), and full scar (anchor shaped) mastopexy depending on your breast shape and tissue quality. Patients that require special mention are those that have lost massive amounts of weight after lap band or gastric bypass surgery. There are numerous techniques that are designed specifically for breast that undergo the significant deflation that occurs with weight loss. Many require "auto-augmentation" or techniques that swing in your own fatty tissue from adjacent areas to fill and lift the breast.

Breast Reduction

Large breasts may cause physical and mental discomfort and can even harm the women who have them -- the size and weight of large breasts can result in self-consciousness, improper posture, pain in the back and neck, indentations from bra straps, skin rashes, breathing problems and skeletal deformities. Breast reduction surgery is usually done to provide physical relief from these symptoms, not for cosmetic reasons. Performed under general anesthesia, the two-to four-hour procedure removes fat and glandular tissue and trims resultant excess skin to produce smaller, lighter breasts that are in a healthier proportion to the rest of the body. Advanced techniques are available to create a beautiful breast shape. It is important to have an artistic sense in breast shaping because rather than simply removing excess breast weight, it is critical to sculpt and shape what is left behind.

Male Breast Reduction

Although rarely discussed, enlarged male breasts is a common condition. Gynecomastia affects approximately 40 to 60 percent of men. There is often no known cause for gynecomastia, although there has been some correlation with various drugs or medical conditions.

Men who feel self-conscious about their appearance are helped with breast-reduction surgery with either liposuction, surgical removal of excess breast tissue and skin, or a combination of the two techniques. These procedures removes fat and/or glandular tissue from the breasts, and in certain cases removes excess skin, resulting in a contoured chest that is flatter and firmer. Men who have lost massive amounts of weight may need formal breast reduction surgery and chest sculpting.

Breast Reconstruction

Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately
following mastectomy, so that the patient wakes with a new breast mound. Alternatively, reconstruction may begin years after mastectomy depending on timing of radiation and/or chemotherapy regimens. There are several ways to reconstruct the breast, both with and without implants; Dr. Ghavami will work closely with you in deciding which is the best for you. Breast reconstruction does not affect the recurrence of cancer, does not cause any other diseases, and does not affect chemotherapy or radiation treatment, when done properly. Dr. Ghavami has completed specialized microsurgical training and is experienced in microsurgical breast reconstruction techniques which gives patients an option to use their own body's tissue to shape and form a natural breast.

Nipple Reconstruction

Nipple reconstruction can provide a boost in self-confidence to patients who have inverted or stretched nipples or who lack one or both nipples due to mastectomy (surgical removal of the breast), injury or a congenital defect. It is often the final step in breast reconstruction surgery, where the nipple is created after the breast mound has settled into place so that the new nipple matches the appearance and position of the natural one as closely as possible. Nipple reconstruction may be performed using one of several techniques, including tissue and cartilage flaps, skin grafts, local rearrangement of the skin on the reconstructed breast, and tattooing.
