Payment Terms

All payments (other than those made through financing or insurance) related to the rhinoplasty procedure must be paid in full two weeks prior to the surgery date, otherwise a penalty fee will be incurred.

In instances where someone other than the patient is paying for the rhinoplasty procedure and/or related fees, they must provide a signed and dated credit card authorization along with a copy of the credit card (both front and back), as well as producing original, legal identification.

Insurance

Dr. Ghavami is not in-network with any insurances, and rhinoplasty will not be covered by your insurance unless it is deemed medically necessary, a decision made on a per-patient basis. If the surgery is deemed medically necessary, certain PPO plans that include an out-of-network benefits option for specialists or surgeons may be used. In those cases, it is important to note that the related fees (for anesthesia and the use of an outpatient surgical center) may or may not also be covered by insurance.

Fees

Dr. Ghavami charges a fee for his pre-surgery consultation, though this fee will be applied to your surgical expenses should you decide to have the surgery.
